  1. My Juniper SRX380 suddenly goes dark, and upon rebooting, I keep encountering the following errors:

    Timecounter "mips" frequency 1500000000 Hz quality 0
    ad0: 95396MB at ata0-master UDMA133 SATA 3Gb/s
    WARNING: Expected rawoffset 187667424, found 187667392
    random: unblocking device.
    hwpmc: OCTEON/4/64/0x1ff
    Trying to mount root from ufs:/dev/ad0s1a
    WARNING: / was not properly dismounted
    cpuid = 0
    ADDRESS_ERR: pid 1 (preinit), uid 0: pc 0x4000e0 got a write fault at 0xfffccab0
    Trapframe Register Dump:

    When I try a clean install from USB, I get the following errors:

    can't load '/kernel'
    can't load '/kernel.old'

    please propose me solutions.
